Lead the design, execution, and evaluation of population health and quality improvement strategies to improve clinical outcomes, address social determinants of health, optimize value-based care, and enhance patient satisfaction across Promise Healthcare.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Strategic Leadership & Program Oversight: Lead PHC's population health strategy and supervise related staff and interns.
- Quality and Value-Based Care Initiatives: Use advanced data analytics to identify gaps, guide decision-making, and monitor impact. Drive quality improvement initiatives to close care gaps, boost preventive screenings, and improve performance on key measures.
- Community Partnerships & Equity: Collaborate with clinical leaders, Managed Care Organizations, and community partners to promote health equity and culturally responsive care.
- Administrative and Supervisory Responsibilities: Oversee development of dashboards and reports and represent PHC in external collaborations. Support grant development and ensure continuous professional development of team members.
QUALIFICATIONS / EDUCATION / EXPERIENCE REQUIREMENTS:
- Master's degree in Public Health, Health Administration, Nursing, Social Work, or related field (or equivalent experience).
- Minimum 5 years in healthcare or public health, with at least 2 years in leadership.
- Strong knowledge of population health, value-based care, health equity, and QI strategies.
- Strong skills in analyzing population health data, using NextGen EHR reports, and turning complex data into clear, practical actions to improve quality of care.
- Bilingual (English/Spanish) preferred.
- Preferably has experience working in FQHCs or similar community-based healthcare settings serving underserved populations.
